We are committed to making the experience of healthcare referrers as seamless as possible for you and your patients. All referrals must be from either a GP or specialist and include:

- Phone number
- Provider number

Donate to the HammondCare Foundation
As an independent Christian charity, we’re committed to providing care for people in need, especially those facing tough times. Many older Australians have care needs that aren’t covered by Government funding. That’s why the HammondCare Foundation is dedicated to raising vital funds to care for those who others can’t or won’t, regardless of their circumstances or background.
With a small act of kindness, you can ensure they get the support they need. We appreciate your support.
