Getting older doesn't have to mean slowing down

It doesn't matter how complex things have become, or whether you prefer to get help at home, in our residential care, or at one of our hospitals – our physiotherapists, dietitians and other allied health professionals are standing by to support you or your patients.

We can also help when health problems aren't just physical, with Older Person's Mental Health services available in our hospitals.

We've been caring for people for over 90 years, so wherever you're at, whatever you need, our teams have the skills, experience and compassion to help.

How to make a healthcare referral

We are committed to making the experience of healthcare referrers as seamless as possible for you and your patients. All referrals must be from either a GP or specialist and include:

  • The referring practitioner’s address
  • Phone number
  • Provider number

If the referral is from a specialist, the patient’s GP details should also be included with the referral. All referrals must be legally valid as outlined in the Medicare Benefits Schedule (MBS).

Please address your letter of referral to our healthcare specialists. It must be received and dated before your patient starts treatment.

Donate to the HammondCare Foundation

As an independent Christian charity, we’re committed to providing care for people in need, especially those facing tough times. Many older Australians have care needs that aren’t covered by Government funding. That’s why the HammondCare Foundation is dedicated to raising vital funds to care for those who others can’t or won’t, regardless of their circumstances or background.

With a small act of kindness, you can ensure they get the support they need. We appreciate your support.
