      Commonwealth Home Support Programme costs

      HammondCare is a leading provider for the Commonwealth Home Support Programme (CHSP) which provides a helping hand with everyday tasks for people seeking low-level support. It also covers in-home, overnight and other forms of respite care.

      Home Care Package costs

      A Home Care Package (HCP) is a Government-subsidised program for older people with more complex care needs, above what CHSP can provide. The costs below usually come from your Package subsidy, but can also be provided through optional self-funded Private Care.

      Nursing care

      A consistent team of registered nurses will regularly visit you at home to provide clinical care in the comfort of your own home.

      Core advisory and care management services delivered by a specialised care team who are on-hand for initial assessment, care coordination, re-assessment and review of your needs.

      ** There are four levels of Home Care Packages, which are based on each individual’s assessed needs.

      Package management is the ongoing administration and organisational activities associated with ensuring the smooth delivery of your Home Care Package.

      Basic daily fee

      In some cases, Home Care Package costs include a basic daily fee set by the Government. HammondCare At Home no longer charges new clients the basic daily fee.* Instead, you may choose to opt for an additional Private Care Agreement, to extend your home care services outside of your Package.

      Important Information
      • You may be required to contribute to the cost of your care by paying an Australian Government Income Tested Care Fee and/or Basic Daily Fee.

      • The Home Care Package funding amounts are current as of 1 July, 2023. More information on contributions is available on the My Aged Care website.

      Telehealth costs

      HammondCare offers free 24/7 telephone support at 1800 826 166. HammondCare also offers clinical Telehealth support which can provide clinical support over the phone. This service is charged at the following rates:

      How do I access Government support?

      The Australian Government funds aged care to make it affordable and accessible. The first step to accessing Government-funded aged care services is to call My Aged Care on 1800 200 422 to discuss your needs and to arrange an assessment. Alternatively, you can do an eligibility check online.

      Respite Care costs

      Respite provides short-term care in your own home so carers can have a break from their caring role. Rates will vary depending on the day and time of booking.
