Inpatient palliative care services for Northern Sydney have temporarily relocated to Neringah Hospital in Wahroonga.
Our dedicated specialist team continues to provide high‑quality, compassionate palliative care, supported by expanded capacity at Neringah Hospital.
All referral pathways remain unchanged, ensuring there is no disruption for GPs, specialists, or hospital partners who need to refer patients for inpatient palliative care.
Community Palliative Care and Healthcare
HammondCare Pacific Centre
Community Palliative Care, Ambulatory, Bereavement, Pain, Telehealth and HCHC Allied Health services have moved to HammondCare Pacific Centre at 38 Pacific Highway in St Leonards.
The new site has all the amenities of Greenwich in newly refurbished rooms and an accessible and convenient location.
Older Peoples' Mental Health
HammondCare Riverglen, Building 50, Macquarie Hospital
Older People's Mental Health (OPMH) services have relocated to HammondCare Riverglen at Macquarie Hospital. The familiar faces of our expert multidisciplinary team ensure continuity of care in a new purpose‑designed space, supporting clinical needs and patient wellbeing.
Neringah Hospital

Braeside Hospital is a public hospital that provides specialised health and community services, including palliative care, in Fairfield.
Palliative Care
Braeside Hospital offers comprehensive palliative care services for people diagnosed with a life-limiting illness when relief from symptoms or pain management may be needed. These services include:
- inpatient unit
- outpatient clinics
- community assessments
- after-hours phone advice for people registered with the palliative care service.
Rehabilitation
We offer comprehensive inpatient and outpatient services to aid recovery and regaining independence for patients who are experiencing a wide range of conditions, including:
- recent stroke
- joint replacement
- lower limb amputation
- fractures.

Care is provided for inpatients in a structured and safe environment with special design features to reduce confusion and promote calm.

Palliative Care
Neringah Hospital has a multidisciplinary team approach to palliative care. This means we embrace the physical, emotional, social and spiritual needs of you and your family.
Our team includes:
- palliative medicine specialists and nurses
- physiotherapists and occupational therapists
- social workers and activity officers
- pastoral care workers and more.
Inpatient Palliative Care unit
Neringah’s inpatient unit provides short-term acute care to patients managing a life-limiting illness. This covers:
- short-term admission to alleviate symptoms and the distress they cause
- holistic care to meet the special needs of patients in the final stages of their illness.
While Neringah Hospital accepts public hospital patients, patients with private health insurance can elect to be admitted as a private patient. If you use your private health insurance hospital cover, you will have no out-of-pocket expenses.
Community Palliative Care
This service helps people manage life-limiting illnesses in the home environment and clinic setting, including:
- home visits by our specialist team
- medical support for symptom control
- supply of specialist equipment to help patients remain at home through the free Equipment Loan Pool
- information and advice on advance care planning.
Patients who are well enough can attend the hospital for appointments with palliative care specialists.
A referral from your GP or treating specialist is required. There are no out of pocket expenses for this service.
