HammondCare Pacific Centre provides specialised health and community services for the residents of Northern Sydney.
Our services include community palliative care, outpatient rehabilitation, pain management, bereavement and home care allied health.
Quality & Safety
We are committed to providing high quality, safe and relationship-based care for our clients and patients. Our hospitals are fully accredited via the Australian Council on Health Care Standards (ACHS) under the National Safety & Quality Health Service (NSQHS) Standards.
We use various strategies and structures to monitor patient safety and quality of care. These include reviews, audits, surveys and feedback from consumers, carers and staff. As part of our ongoing quality and safety culture we perform regular clinical indicator reviews.
In addition to internal and external professional reviews, we have a Consumer Advisory Council that reviews patient safety data and provide comments on a regular basis.

We provide palliative care for patients and their families living at home on Sydney’s North Shore and Northern Beaches.
Our palliative care aims to make patients with a life-limiting illness as comfortable as possible, while expertly managing symptoms.
We offer support that embraces physical, psychological, social and spiritual needs for the patient, their family and other carers.
Our rehabilitation services offer personalised treatment designed to help you adjust and rebuild your life so you can live as independently as possible.

Our pain management service offers a telehealth phone or video consultation to people living in Far Western NSW. The pain management team can provide help and support to improve pain-related conditions.
We support you to stay well and independent at home as you age. Our Restorative Care team, made up of allied health practitioners, can provide advice and support to help you stay well and independent at home, so you can continue to do the things you love.
We provide free, confidential grief and bereavement counselling, education and support groups for individuals and families. You can register yourself or someone you care for.
