Medibank

HammondCare and Medibank have partnered in a pilot program to provide patient choice and flexibility when it comes to where they receive palliative care.

This program provides comprehensive support for the patient, their family, and carers, enabling patients to receive specialised end-of-life care in their home environment. We offer personalised support that embraces physical, psychological and spiritual needs, coordinated by a specialist palliative care clinician.

Bupa

The Palliative Care Choices program provides care and support for Bupa patients in the location of their choice.

A range of multidisciplinary clinicians, including specialised carers, are available to provide home-based support for patients, families and/or carers.

Services provided to patients are based on ongoing assessment of individual need and preference, including medical, nursing, psychosocial support, respite, personal care and allied health support. The primary goal of the program is to provide a holistic personal experience tailored to individual needs and provided in the comfort of patients’ own homes.

HammondCare’s Palliative Centre has partnered with the Sydney North PHN to deliver the quality end of life care in the Residential Aged Care Homes program. HammondCare is working with a group of 34 residential aged care facilities in Northern Sydney to build capacity and confidence of nursing, and care staff to provide quality end of life care to residents. Phase Four will continue to provide education and include the introduction of the Palliative Care Outcomes Collaboration (PCOC) routine assessment and screening tools, enabling appropriate and timely response to patient and carer needs. 

HammondCare and Sydney North PHN have also partnered to develop a pilot course to provide skills and confidence to help a person in the last months, weeks and days of their life. The Last Days Pilot project aims to provide simple and effective education to carers, family, friends and community about palliative care and to enhance public discussion about death and dying, by improving awareness, creating engagement and strengthening the community’s ability to be a more death literate society. The course has been developed in consultation with community members with life experience, with four pilot sessions planned for late 2020

In 2019, the Adelaide PHN commissioned HammondCare Community Palliative Care to deliver training to residential aged care staff to build their capacity and confidence in providing quality end of life care. The training focused on advance care planning, effective communication, and early identification and assessment of deterioration. This funding was part of the Greater Choices in Palliative Care at Home funding round (2018), Enabling Choice for South Australians.

In 2019, HammondCare provided face-to-face workshops on palliative care and advance care planning in residential aged care to three regional areas in the Central Queensland PHN.

In 2019, The Dementia Centre and HammondCare were commissioned to provide face-to-face workshops to residential aged care homes on palliative care and advance care planning.

In 2019, the Western NSW PHN in partnership with the SHARE Project commissioned HammondCare to create an educational video addressing advance care planning education in residential aged care.

  • Northern Sydney LHD | HammondCare works in partnership with the LHD and PHN to provide palliative care across the district. Inpatient specialist palliative care is provided through in-reach consultative services to acute hospitals, and in the two specialist palliative care units at Greenwich and Wahroonga. Specialist palliative care is provided in the community through integrated care arrangements with Sydney Home Nursing and GPs.
  • Southern NSW LHD | HammondCare has partnered with Southern NSW Local Health District to deliver an outreach service provided by our Palliative care medical staff specialists. This service includes bimonthly visits to agreed areas across the LHD to deliver specialist medical clinics and education, as well as telehealth support.
  • South West Sydney LHD | HammondCare’s Braeside Hospital resides in the South West Sydney LHD. Palliative care services provided include an inpatient unit, day hospital, outpatient clinics, community assessments and after-hours telephone advice for people registered with the palliative care service.
  • Palliative Care Home Support Program | HammondCare leads a consortium to deliver specialised end of life care to people in seven Local Health Districts in NSW. The Palliative Care Home Support Program (PCHSP) has been operating since 2013 and supports people to die in their place of choice, which for most Australians is at home. Trained community care workers support both patients and their carers with personal and respite care in the last days of weeks of life. The program operates in the following LHDs:
  • Northern Sydney LHD
  • South East Sydney LHD
  • Western NSW LHD
  • Far West LHD
  • Southern NSW LHD
  • Central Coast LHD
  • Murrumbidgee LHD

Since its inception, the program has supported close to 5,000 people, with more than 70% of these patients dying at home.

Specialist Medical in Reach

For over 30 years, HammondCare has a history of partnering with hospitals to provide palliative and end of life care services where they would otherwise be lacking. We currently partner with a number of Sydney hospitals to deliver an in-reach service, which is provided by our Palliative Care Medical Specialists and Clinical Nurse Consultants. We provide clinical support, training and education to hospital staff, in addition to advance care planning, discharge planning and bereavement support to families.

Participating Hospitals include:

  • Macquarie University Hospital (nursing)
