HammondCare delivers comprehensive education services tailored to the different groups who may be involved in the care of patients with a life-limiting illness, including healthcare professionals, care workers, family members and loved ones.
Our team of experienced palliative and supportive care professionals steer education programs at community, state and national level. They provide knowledge and guidance across a range of platforms including online, physical resources, and face-to-face mentoring.
As leaders of Palliative Care education, we also partner with healthcare professionals, organisations and industry bodies to source the most up-to-date information, in order to deliver programs that offer best-practice solutions for those in our care.
Last Days Program
HammondCare and Sydney North Health Network have partnered to develop an education course for carers, family, friends and community about Palliative Care. This provides invaluable skills and confidence to assist a person in their last months, weeks and days. The Last Days Program aims to provide simple and practical education.
The Advance Project
The Advance Project is one of the National Palliative Care Projects, funded by the Australian Government since 2016 and led by HammondCare’s Professor Josephine Clayton, working in collaboration with various health organisations and universities across Australia. The Advance Project provides a practical, evidence-based toolkit and a training package, specifically designed to support Australian General Practitioners (GP’s), General Practice Nurses and Practice Managers. It enables general practices to implement a team-based approach to initiating advance care planning and palliative care into everyday clinical practice.
Quality End of Life Care
The Quality End of Life Care in Residential Aged Care Homes project aims to support residential aged care staff in End of Life Care. It provides specialised advice, expertise and training in palliative and supportive care. A major focus is fostering partnerships between aged care and GP’s to deliver better outcomes for patients and families.
