Our Story

HammondCare has been providing End of Life Care for people since 1907, when the Eversleigh Home of Peace was established. 

A focus on continuous innovation, research and dedication to better care for over 100 years has enabled us to become one of Australia’s most innovative providers in the field of palliative and supportive care.

The Palliative Centre was established to provide a central hub for patients, families and healthcare professionals to easily access a wide range of leading research, services, education and resources in the one place.

The Palliative Centre combines all our expertise in palliative care, advance care planning and aged care service provision; along with sharing our successful experience in leading and contributing to local, state and national programs, which has built a strong body of expertise and resources.

Our Strategy

Our Palliative Care Strategy, Reshaping Palliative Care for the Future, provides clear goals and measures to strengthen and broaden our palliative care services, programs and research across the organisation and into the community.

The five-year Strategy outlines four critical goals:

  1. Enable people to die with dignity
  2. Improve palliative care equity and access for vulnerable communities
  3. Improve knowledge, skills and confidence in palliative care for the workforce and community
  4. Lead the health and aged care sector in integrated services and palliative care research

This Strategy empowers and respects the choices of people at the end-of-life, and strengthens our approach to deliver timely, quality care wherever the client is.

Current day

Today, our specialist palliative care services work in partnership with two Sydney-based Local Health Districts. 

This strengthened network of medical specialists, multidisciplinary palliative care teams, inreach assessment services and expanded capacity in the inpatient care units, enables us to provide unparalleled End of Life Care. 

HammondCare also provides a range of residential, home and community based aged care services. This combined capability in subacute and aged care settings allows a continuum of care as people journey through chronic and end-stage illness. 

With palliative and aged care services under the same governance structure, HammondCare has a unique ability to develop innovative models of care that best support each person, no matter where they receive care.

What does it mean to champion life when caring for someone at End of Life?



HammondCare and Medibank have partnered in a pilot program to provide patient choice and flexibility when it comes to where they receive palliative care. 

This program provides comprehensive support for the patient, their family, and carers, enabling patients to receive specialised end-of-life care in their home environment. We offer personalised support that embraces physical, psychological and spiritual needs, coordinated by a specialist palliative care clinician.

This pilot program provides care for patients who wish to remain in their home during End of Life Care. A range of multidisciplinary clinicians are available to provide home-based support to the patients and their families and/or carers. 



The Palliative Care Choices program provides care and support for Bupa patients in the location of their choice.

A range of multidisciplinary clinicians, including specialised carers, are available to provide home-based support for patients, families and/or carers.

Services provided to patients are based on ongoing assessment of individual need and preference, including medical, nursing, psychosocial support, respite, personal care and allied health support. The primary goal of the program is to provide a holistic personal experience tailored to individual needs and provided in the comfort of patients’ own homes.

Sydney North Health Network 

HammondCare’s Palliative Centre has partnered with the Sydney North Primary Health Network (PHN) to deliver Quality End of Life Care in RACH. In partnership with the PHN, HammondCare is working with a group of 26 residential aged care facilities in Northern Sydney to build capacity and confidence of nursing and care staff to provide quality End of Life Care to residents. 


Adelaide Primary Health Network 

In 2019, the Adelaide Primary Health Network commissioned HammondCare Community Palliative Care to deliver training to residential care staff to build their capacity and confidence in providing quality End of Life Care. This funding was part of the Greater Choices in Palliative Care at home funding round (2018).


HammondCare is driven by our mission to improve quality of life for people in need. We provide daily support for people in need facing the hardest times life has to offer, regardless of religion, position or circumstance. We would not be able to deliver the palliative and supportive care services we do without the support of our generous donors.
