How do I access these services?

How do I access these services?

The Palliative + Supportive Care Inpatient Units can provide; - Short-term admission for symptom management and/or; - Admission to meet the special needs of patients and their families and carers in the final stages of their journey.

A referral from a general practitioner or treating specialist team is required for admission to our Palliative Care service.

Inpatient Units are at Neringah Hospital, Greenwich Hospital, and Braeside Hospital.

Community palliative care services are available within Ryde, Sydney's Upper and Lower North Shore and the Northern Beaches.

Palliative Care Home Support Program collaborates with each local health district, local community care providers and the Paediatric Palliative Care Network to help you stay at home during a life-limiting illness.
