Palliative Care Health Insurance Partnerships with HammondCare

HammondCare partners with a number of private health insurers to offer End of Life Care for members to ensure the best possible care within their own home.

HammondCare and Medibank

HammondCare and Medibank have partnered in a program to provide patient choice and flexibility when it comes to where they receive palliative care. This program provides comprehensive support for the patient, their family and carers, enabling patients to receive specialised End of Life Care in their home environment. We offer personalised support that embraces physical, psychological and spiritual needs, coordinated by a specialist palliative care clinician.

This program provides care for patients who wish to remain in their home during end of life care. A range of multidisciplinary clinicians are available to provide home-based support to the patients and their families and carers.

Services tailored to individuals' needs

Services provided to patients are based on ongoing assessment of individual need and preference, including medical, nursing, psychosocial support, respite, allied health and equipment provision. A bereavement care follow-up service is also available to carers and families.

The primary goal of the program is to provide a holistic personal experience tailored to individuals' needs and provided in the comfort of their own home.
