How do I access these services?

The Palliative + Supportive Care Inpatient Units can provide; 

  • Short-term admission for symptom management and/or;
  • Admission to meet the special needs of patients and their families and carers in the final stages of their journey.

A referral from a general practitioner or treating specialist team is required for admission to our Palliative Care service.

GPs and health professionals can refer their patients to the service by completing the relevant referral form.

All referrals must be legally valid as outlined in the Medicare Benefits Schedule (MBS) and include your letter of referral to our staff specialists.

Three simple steps to obtain a referral for HammondCare's Palliative + Supportive Care services

1. Referral - Ask your GP or specialist about how palliative and supportive care might benefit you, and ask for a referral.

2. Assessment - our team will schedule an initial assessment at one of our clinics or your home.

3. Care plan - our team will create a personalised care plan recommending support based on your needs.
