Medibank Palliative Care At Home Pilot Program

HammondCare and Medibank have partnered in a pilot program to provide patient choice and flexibility when it comes to where they receive palliative care. This program provides comprehensive support for the patient, their family, and carers, enabling patients to receive specialised end-of-life care in their home environment. We offer personalised support that embraces physical, psychological and spiritual needs, coordinated by a specialist palliative care clinician.

This pilot program provides care for patients who wish to remain in their home during end of life care. A range of multidisciplinary clinicians are available to provide home-based support to the patients and their families and/or carers.

Clinicians available through this program include:

  • Palliative care physicians
  • Registered nurses
  • Physiotherapists
  • Occupational Therapists
  • Social workers
  • Bereavement counsellors
  • Palliative care trained care workers

Services tailored to individuals' needs

Services provided to patients are based on ongoing assessment of individual need and preference, including medical, nursing, psychosocial support, respite, allied health and equipment provision. A bereavement care follow-up service is also available to carers and families. 

The primary goal of the program is to provide a holistic personal experience tailored to individuals' needs and provided in the comfort of their own home.

In the Palliative Care At Home Pilot Program, nurses are available seven days a week from 8am – 4:30pm with nursing and medical support available over the phone if concerns arise after these hours. Specialised care workers are also available to provide personal care and respite as required.

Medibank has partnered with HammondCare to provide this pilot program to Medibank members who reside in Northern Sydney, hold an eligible Medibank hospital insurance policy, and meet eligibility requirements.

If this is of interest, we suggest you consult your treating doctor to see whether home-based treatment may be right for you. You must have eligible hospital cover with Medibank and have served all relevant waiting periods. Your treating doctor must also consider it appropriate for you.

Medibank funds the cost of at-home palliative care services. However if you require additional treatment, such as diagnostics services or a follow up with your oncologist, then there may be related out of pocket expenses. As with any treatment, it’s best to give Medibank a call beforehand on 1800 065 769 to check what’s included in your policy and also to consult any doctors involved in your treatment about their fees.
