HammondCare + Medibank Palliative Care At Home Pilot Program

  • HammondCare and Medibank have partnered in a pilot program to provide patient choice and flexibility when it comes to where they receive palliative care.
  • The program provides comprehensive support for the patient, their family and carers, enabling patients to receive specialised end-of-life care in their home environment.
  • We offer personalised support that embraces physical, psychological and spiritual needs, coordinated by a specialist palliative care clinician.
  • A range of multidisciplinary supports, including specialised carers, are available to provide home-based support to the patient and family.
  • Medibank is funding HammondCare to provide this pilot program to Medibank members who reside in Northern Sydney, hold an eligible Medibank hospital insurance policy, and meet eligibility requirements.

Inpatient Units are at Neringah Hospital, on Sydney's Upper North Shore and Greenwich Hospital, on Sydney's Lower North Shore, and Braeside Hospital in South West Sydney.

Community palliative care services are available within Ryde, Sydney's Upper and Lower North Shore and the Northern Beaches.

How do I access these services?

The Palliative + Supportive Care Inpatient Units can provide; 

  • Short-term admission for symptom management and/or;
  • Admission to meet the special needs of patients and their families and carers in the final stages of their journey.

A referral from a general practitioner or treating specialist team is required for admission to our Palliative Care service.

GPs and health professionals can refer their patients to the service by completing the relevant referral form.

All referrals must be legally valid as outlined in the Medicare Benefits Schedule (MBS) and include your letter of referral to our staff specialists.

1. Referral - Ask your GP or specialist about how palliative and supportive care might benefit you, and ask for a referral.

2. Assessment - our team will schedule an initial assessment at one of clinics or your home.

3. Care plan - our team will create a personalised care plan recommending support based on your needs.

How much does palliative care cost?

HammondCare is subsidised by State Government funding, making our services accessible at public hospitals. This means there is no cost for patients to use our Palliative & Supportive Care services. When you choose to use your private health insurance, there are no out of pocket expenses.
