Your step-by-step guide

Step 1

Arrange an assessment
via My Aged Care

Find out if you can be assessed for a government-subsidised place in residential care. To get started, contact the Australian Government department for aged care, My Aged Care. If you are eligible for an assessment, you will receive a letter with referral codes.

Step 2

Find a care home that
interests you

Research our residential care homes near you and take a virtual tour. When you have a location in mind, contact us for a face-to-face
assessment with our expert care team.

Your first assessment through My Aged Care is for government funding. Your assessment at HammondCare is to determine the care you need.

Step 3

Take a guided tour in person

Come for a guided tour with one of our friendly team members. Our team will help you understand key features of our homes as well as the costs and payment options.

We'll provide you with an application pack to complete if you would like to move in.

Step 4

Move in to your new home

When a suitable place becomes available, it’s time to move in. We’re here to help to make the transition into care as smooth and stress-free as possible. Our care team will already be familiar with you through our unique assessment process.
