Getting the most from your home care funding

If you haven’t reviewed your aged care funding recently, you might be surprised to learn about additional services available to you.

HammondCare At Home works in partnership with you and your family, to maximise your home care funding and arrange the services best suited to you and your needs.

Over time, many people’s needs change, and so it’s important to review your care plan together. You can utilise HammondCare’s years of experience and expertise, by consulting your dedicated care team, to find what’s right for you now.

You’ll be fully supported from the get-go and will be supported by your local care team, who can help to advise what services are available to you – including complimentary services such as pastoral care and volunteer services.

As a reminder, home care services fall into three broad categories.

Services to keep you well and independent:

  • Personal care including help with getting in and out of bed, bathing and dressing.
  • Nursing services, including wound care and management, and help with medication.
  • Allied health including podiatry, physiotherapy, hearing and vision services.
  • Food and nutrition services, including meal preparation and help with eating.
  • Specialised support including continence and dementia advice and support for people from culturally and linguistically diverse backgrounds.

Services to keep you safe in your home:

  • Home duties including making beds, vacuuming and light gardening.
  • Minor home modifications including easy-access taps, and grab rails in the bathroom.
  • Aids and equipment including walking aids, bed rails and slide sheets.

Services to keep you connected to your community:

  • Transport services so that you can get to appointments and community activities including help with shopping, visiting doctors and attending social events.
  • Social support services including in-home visitors, companions for outings or someone to set up your phone and internet services to help keep loved ones close.

HammondCare also offers a variety of Positive Ageing programs providing skills and knowledge to improve your quality of life. Our evidence-based programs can help you meet your nutrition needs, maintain balance and strength so you can live at home confidently, happily and explore your creativity to boost your wellbeing.
