We do nursing and clinical care, by experts

Our professionally qualified clinical care managers can come to your place and manage medications, wound care, post-op care, injury support and more.

We do respite care, day or night

You don’t have to go it alone – we offer a little extra care for a few hours, or a few days, with personalised, compassionate respite support at home, or at one of our cottages or day centres.

We do dementia care, that really connects

Our dementia expertise informs every aspect of our care, with home care teams skilled in support for people living with dementia, and specialist advice available.

We do physiotherapy, and more

Getting older doesn’t have to mean slowing down – our physiotherapists, dietitians and other allied health professionals are standing by to support you.

We do a little home help, inside and out

Our team can help with practical, day-to-day tasks such as meal preparation, grocery shopping, and keeping your home and garden tidy and safe.

We do pastoral care, whatever you believe

As an independent Christian charity, we offer emotional and spiritual support to people of all faiths, or none, from any cultural background: it’s about what matters to you.

We do palliative care, with compassion

However complicated things get, our home care teams can support people (and their families) facing a life-limiting illness to live in their own home, as well as possible, to the very last days.

We do volunteers, for great company

Whatever you love, there’s probably a HammondCare volunteer who loves it, too. Our 650+ volunteers are looking forward to spending time with you, enjoying your hobbies, activities and interests.

We do social clubs, because connection matters

Step out of your routine at one of our Social Clubs, while your carer takes a break. Chat with people your own age, take part in activities, and share in a delicious, homemade lunch.

Help with activities of daily living, and managing the loss of skills required to live independently.

A co-payment between 5% - 50% may apply based on pension status and funding approval date*.

* If you were approved for a Home Care Package on or before 12 September 2024, you are eligible for the Government's no worse off principle which guarantees that your financial contribution will be the same or less than what you were required to pay under the Home Care Package program.

Support at Home is a Government-subsidised program that helps you live independently at home for longer.

A Support at Home package offers support including clinical supports, independence supports and everyday living, all with the goal of making living at home as easy and comfortable as possible.

Support at Home services will be divided into three categories:

  • Clinical Supports: specialised services to help you maintain or regain functional and/or cognitive capabilities.
  • Independence Supports: support to help you manage the loss of skills required to live independently, and help with activities of daily living.
  • Everyday Living: Support to help you keep your home in a liveable state so you can stay living independently for as long as possible.

Support at Home has been designed as a "pay for what you use" system. That means you'll contribute to the cost of certain services, if you're assessed as having the means to do so. A lifetime cap applies, so the amount you contribute is limited. If you move into residential care, this cap travels with you.

What you contribute will depend on your assessed income and assets as determined by Services Australia.

If you had a home care package prior to 1 November 2025, or approval for funding, grandfathering or transitional conditions may apply to you.
