Community Aged Care Package (CACP)

This program provides a planned and managed package of community care if you have complex low-level care needs but can still live in your own home.

Extended Aged Care at Home (EACH)

Extended Aged Care at Home (EACH) is a program that provides you with nursing home level care at home if you need more assistance than a Community Aged Care Package can provide. EACH packages are also individually planned and coordinated for you.

EACH Dementia

If you or someone you care for needs assistance because of behavioural problems associated with dementia, including periods of changes in behaviour, the EACH Dementia program can provide high-level care through an individually tailored package.

Residential Aged Care

At Horsely we will offer low and high care dementia specific accommodation. For more information about these facilities click here. HammondCare’s newest dementia specific facility is based at Horsley (near Wollongong), providing hostel and nursing home care for 90 residents.

HammondCare believes that people living with dementia should have every opportunity to live as independently as possible, function at their greatest potential and enjoy life. HammondCare Horsley is unique. Made up of six cottages with all the features of a domestic home, HammondCare Horsley offers a calm environment where residents can feel at home. At HammondCare Horsley we are committed to caring holistically for people living with dementia.

Services for younger people living with dementia at HammondCare Horsley

HammondCare also offers a residential and residential respite service specifically for younger people with dementia at HammondCare Horsley. Residential and residential respite places are usually offered in Streeton cottage, one of the six cottages on the Horsley site. Streeton cottage is specifically designed for people with dementia.
