Palliative Care Outcomes (PCOC) update

A fantastic achievement in 2020 has been the implementation of the Palliative Care Outcomes Collaboration (PCOC) data collection across five facilities: Opal Lourdes, BaptistCare Shalom Centre, Uniting Wesley Heights, Twilight Horton House and Caroline Chisholm Hall & Prior.

Recognising the success of this symptom observation and management approach, one of the senior managers of the facilities, who had already implemented this program, has rolled it out across four more locations.

PCOC aims to improve palliative care patient and carer outcomes. It has been proven to be a successful model to drive systems-level improvements across different care settings with older adults.

In the next phase of the Quality End of Life Care Project (2021-22), we are looking for another five facilities to take part in the quality improvement initiatives. This includes implementation of PCOC in Residential Aged Care (RAC), supported by a Palliative Care Clinical Nurse Specialist (CNS), to conduct Palliative Care Needs Rounds and family meetings/webinars.
