What is a Home Care Package?

A Home Care Package is a Government subsidised service to help older Australian’s live independently at home for longer. The types of support available through a Home Care Package include help with housework, personal care, respite, meals and food preparation, transport, shopping, allied health, social support and more. 

Any older person who is experiencing difficulty managing at home may be eligible for a Home Care Package.

How to access a Home Care Package

There are three steps to accessing a Home Care Package:

1. Check your eligibility

If you have home care needs and are considering accessing a government-subsidised Home Care Package, you will first need to contact My Aged Care by calling 1800 200 422 or visit myagedcare.gov.au.

If your care needs indicate that you might be eligible for a Home Care Package, My Aged Care will organise for an assessment team to complete a free assessment. This face-to-face assessment will identify what your home care needs are and confirm your eligibility to receive government-subsidised aged care services. My Aged Care will write to you to let you know the outcome of your assessment, the level of care you are eligible for and approved to receive, as well as an overview of that care.

2. Find a home care provider

Once you have had your free assessment with My Aged Care and received your My Aged Care approval, your name will be placed on the national package waitlist. This is when you can start looking for an approved home care provider to provide your Home Care Package. Home care providers are not all the same; they offer different services and have different types of services and fees. It is important to do your research and understand what is included with your chosen provider, and what is not.

The time it takes to receive My Aged Care approval can vary depending on the length of the national package waitlist, however, there are options available through HammondCare, which will allow you to access care shortly after you have had your assessment. 

3. Design your Home Care Package

When you nominate HammondCare At Home as your preferred home care provider, we will get in contact with you to organise a visit in your home so you can get to know us and so we can understand your individual needs. Together, we can develop the best care plan and services to suit you. If you like, we can also include your family or friends in any decisions made. 

If you require more extensive care than is specified in your government-subsidised Home Care Package, we can offer you additional Private Care services until My Aged Care is able to refer you to a higher-level Home Care Package.

The range of care and services available under a Home Care Package includes, but is not limited to:

  • Personal care including help dressing, showering or bathing
  • Social support and companionship
  • Nutrition and meal preparation 
  • Continence management
  • Mobility and dexterity support to remain mobile
  • Nursing care and other clinical services
  • Allied health support including Podiatry and Physiotherapy
  • Transport and personal assistance to help you get out and about i.e. appointments or groceries etc.

How a Home Care Package can help

A Home Care Package can help you to live comfortably and independently at home. One of the extra benefits is that your package can be adapted to meet your changing needs over time.

For example, as a Level 1 Home Care package recipient, you may receive four hours per fortnight of bathing, hygiene and grooming as well as cleaning and laundry services. However, after six months you may decide to include food preparation services and less laundry services.

Regular reviews with your dedicated care team will ensure you’re receiving the best possible care.

Help with staying well

These are the services that help you stay well, active and independent, including personal care, medication, help with meals and access to therapies such as podiatry and physiotherapy.

Help with staying connected

You can enjoy a range of options for visits, social activities and other ways to feel part of the community, including attendance at nearby HammondCare social clubs.

Help around the home

From dusting and vacuuming to having someone do your shopping, you can receive help with most everyday tasks.

Help with nursing and respite care

Specialised nurses can help to oversee your treatment and care, while respite care can range from staying overnight or longer in a specialised respite cottage to regular day care in the community.

Positive ageing
