A better way to live with dementia

We believe that the best place for people with dementia to be is home. When this is no longer possible, then our commitment is to provide an environment that looks, feels and operates like a home.

HammondCare’s first residential aged care facility was built at Hammondville in Sydney’s South-West. It is now HammondCare’s largest site, and includes four residential aged care homes, all specialising in dementia care and focusing on different care needs.

Residents are encouraged to live and operate in the cottage as they would in a home; the resident is free to spend time in any of the common areas, and the buildings are designed so each choice is a ‘right’ one. This means there are no dead-ends and any locked doors are unobtrusive. There are many visual ‘cues’ about where one is, and where one is heading. To minimise confusion, the central kitchen and living areas are always visible from corridors or other spaces.

Bond House

HammondCare’s Bond House is a mixed low and high-care dementia home. It is made up of Jones Hostel, Shaw and Poate Homes, and Lavender Palliative Care Suite.

A low-care home, with large and recently refurbished single rooms. Each room has an ensuite, and residents also have access to a number of common areas.

These common areas include:

  • A main dining room, with small dining tables and chairs for comfortable group dining, a piano, and a deck with outdoor furniture
  • Two large verandahs, with outdoor furniture and a range of pot plants
  • The Burt Sitting Room, with bookshelves, a TV, DVD player, tables and chairs, and a number of comfortable lounge chairs
  • A smaller wing dining room, containing small tables and chairs for comfortable group dining
  • A visitors’ room with a table and chairs, and a kitchenette that is fitted out with a fridge, toaster and microwave
  • A TV room with another TV and large table and chairs for activities
  • A sitting room with desks, two computers and comfortable lounge chairs
  • A common room with comfortable lounge chairs

Between them, Shaw and Poate nursing homes have 63 high-care places, all with large ensuite rooms.

There are a range of common areas including:

  • Poate Sitting Room, which has a TV and comfortable lounge chairs
  • Poate Dining Room, with small tables and chairs that can be rearranged for comfortable group dining
  • A small sitting room with a TV, an organ, lounge chairs and room for wheelchairs and water chairs
  • An outdoor courtyard, which has ramp access, a well-maintained garden, a deck with benches and tables, a barbecue and a shaded area
  • The Shaw Dining Room, with small tables and chairs, comfortable for group dining
  • The Shaw Activities Room, which has a TV, a piano and chairs
  • The Shaw Sitting Room, which has tables and chairs

Lavender Palliative Care Suite is a specialist palliative care unit with ensuite rooms and a sitting room, containing comfortable lounge chairs, a kitchenette with coffee-making facilities, and a TV.

The Lavender Suite is supported by the Specialist Palliative Care team of Braeside Hospital and South West Sydney Local Area Health District.

We aim to provide comprehensive support for the person, their family and carers, support which embraces physical, psychological, social and spiritual needs. We value understanding your wishes for care in the future, and can provide information for you about how to communicate those wishes to your care team.

To be eligible for referral to the Lavender Suite, you will need to have both a specialist palliative care assessment and an Aged Care Assessment. Our Client Admissions Team would be pleased to answer any questions regarding the admission process.

Bond House also has a number of common areas and facilities that all residents can utilise.

These common areas include:

  • A Games room with a pool table, a putting green, an organ and a kitchenette.
  • A chapel
  • A dental room
  • A physiotherapy room
  • A domestic laundry with a washing machine, a drier and an ironing board that residents are able to use as they wish

All areas of the facility are accessible, with wide corridors and call points located throughout the home.

Southwood

HammondCare Southwood has been designed using internationally recognised principles to promote choice, minimise confusion and maximise independence for people living with dementia.

The six small and domestic cottages, all with ensuite bathrooms, within HammondCare Southwood have a variety of common spaces.

HammondCare Southwood contains the following common spaces:

  • A fully functioning domestic kitchen
  • An open plan lounge and dining area
  • Two sunrooms containing lounge chairs and small tables located at the ends of each cottage, each with views to the garden and with good natural light
  • A parlour containing tables and chairs to provide a private break-out space for residents.
  • A TV room with a fireplace
  • A backyard contain barbecues, benches and tables, raised garden beds and washing lines. Secure and looping garden paths link the outdoors with the indoors, minimising confusion for residents
  • A domestic laundry with a washing machine, a dryer, a basin and a clear door so residents can see what is happening inside
The Meadows

HammondCare’s The Meadows is a low-care dementia home consisting of three small, domestic cottages with ensuite bathrooms.  

Residents in each cottage of The Meadows access to the following common spaces:

  • A fully functioning domestic kitchen
  • An open plan lounge and dining area
  • Two sunrooms containing lounge chairs and small tables located at the ends of each cottage, often with views to the garden, and have good natural light
  • A parlour containing tables and chairs to provide a private break-out space for residents. They also have a fold-out bed and an ensuite which family members can use, should they wish to stay the night
  • Two backyard areas contain barbecues, benches and tables, raised garden beds and washing lines
  • A domestic laundry has a washing machine, a dryer, a basin and a clear door so residents can see what is happening inside
The Pines

HammondCare’s The Pines is a low-care dementia home consisting of three small, domestic cottages.  The three cottages within The Pines contain a variety of common spaces, allowing residents to choose where and with whom they spend their time.

Residents in each cottage of The Pines have access to the following common spaces:

  • A fully functioning domestic kitchen
  • An open plan lounge and dining area
  • Two sunrooms containing lounge chairs and small tables located at the ends of each cottage, often with views to the garden, and have good natural light
  • A parlour containing tables and chairs, and provide a private break-out space for residents. They also have a foldout bed and an ensuite which family members can use, should they wish to stay the night
  • Two backyard areas contain barbecues, benches and tables, raised garden beds and washing lines, with pergolas that back onto a duck pond
  • A domestic laundry has a washing machine, a dryer, a basin and a clear door so residents can see what is happening inside

Food preparation and planning is overseen by our Executive Chef and nutritionists, who ensure all the residents’ nutritional needs and tastes are catered to, and that varied, fresh and delicious food is available every day.
