How to access home care services with HammondCare

1. Check your eligibility

If you have home care needs and are considering accessing a government-subsidised Home Care Package, you will first need to contact My Aged Care by calling 1800 826 166 or visit myagedcare.gov.au.

My Aged Care will organise for an assessment team to complete a free Face-to-face assessment, to identify what your home care needs are and confirm your eligibility to receive government-subsidised aged care services. My Aged Care will write to you to let you know the outcome of your assessment, the level of care you are eligible for and approved to receive, as well as an overview of that care.

HammondCare At Home is happy to guide you through the application process.

 

2. Nominate HammondCare At Home as your home care provider

Once you have confirmation of your Home Care Package funding, contact a member of our friendly admissions team, who will be able to connect you with your nearby local home care team.

When you nominate HammondCare At Home as your preferred home care provider, we will get in contact with you to organise a visit in your home so you can get to know us and so we can understand your individual needs. Together, we can develop the best care plan and services to suit you. If you like, we can also include your family or friends in any decisions made.

3. Begin enjoying your Home Care Package

There are a range of care services available through your Home Care Package, which includes access to the following:

  • Personal care including help dressing, showering or bathing
  • Social support and companionship
  • Nutrition and meal preparation 
  • Continence management
  • Mobility and dexterity support to remain mobile
  • Nursing care and other clinical services
  • Allied health support including Podiatry and Physiotherapy
  • Transport and personal assistance to help you get out and about i.e. appointments or groceries etc.

To ensure that you are fully supported to maximise the funding available, we will work with you to assist with all aspects of your Home Care Package management, including arranging regular care plan reviews to evaluate your satisfaction and plan services, provided by your dedicated care team.

Terms & Conditions

  1. This offer is only valid to new HammondCare At Home - Home Care Package clients residing in Queensland or the Port Macquarie and Coffs Harbour Aged Care Planning Regions only.
  2. This offer is only applicable to the Care Management Fees payable under a Home Care Package. At the time of issuing these Terms and Conditions, the amount payable over the 3 month fee free period for Care Management for a Level 1 Package was $359.58, Level 2 Package $632.52, Level 3 Package $1,376.64 and Level 4 Package $2,086.50 respectively.
  3. The ‘3 Months Complimentary Care Management Fees’ for new Home Care Package clients is based on the rates applicable to the level of their Home Care Package Government subsidy and are subject to change from time to time.
  4. The ‘3 Month Complimentary Care Management Fees’ period will begin at the commencement of a client’s Home Care Package Agreement for services provided by HammondCare At Home.
  5. There will be no discount or offset of fees for ‘care and support services’, ‘nursing care’ and ‘in-home respite’ and ‘package management’ provided under a Home Care package or the Basic Daily Fee or Income Tested Care Fee payable under a Home Care package.
  6. Any unused amount of the Care Management Fee is not redeemable for cash and will remain in a client’s Home Care Package until used or forfeited by the client no longer receiving a Home Care Package, whichever occurs first.
  7. A client who transfers their Home Care Package to another provider, will not be eligible for this or any similar offer from HammondCare At Home for 6 months should the client choose to transfer their package back to HammondCare At Home.
  8. This is a limited time offer and HammondCare At Home reserves the right to withdraw this offer to new clients at any time.
