HammondCare is proud to be participating in this year’s National Palliative Care Week, a national event celebrating the important role that palliative care plays in caring for people with life-limiting illness.

At HammondCare, we work to improve the quality of life for people in need and with over 90 years' experience providing a relationship-based approach to care, we’re proven leaders in the field of palliative care.

Reshaping the future

We’ve further developed this year’s national theme to reflect the importance of accessible palliative care where and when it’s needed - “Palliative care: right time, right place – it's your right!”

That’s a key focus of our new five-year Palliative Care Strategy, Reshaping Palliative Care for the Futurewhich establishes clear goals and measures to strengthen and broaden palliative care service delivery and awareness. It also showcases our key services and programs, and shares the stories of people and their families we’ve supported.

Launching on 24 May at Pallister House, Greenwich, the strategy provides direction for our palliative care journey to 2026, motivated by our aim to empower and respect the choices of people at the end-of-life, to deliver timely and quality care wherever the client, resident or patient is. 

The strategy outlines four critical goals:

  1. Enable people to die with dignity by providing an enhanced model of care and supporting their decisions and preferences
  2. Improve palliative care equity and access for vulnerable communities, particularly those with complex dementia
  3. Improve knowledge, skills and confidence in palliative care – for staff across healthcare, home care and residential aged care environments. To increase death literacy in the general community
  4. Lead the health and aged care sector by providing integrated palliative care services and deliver world-class research

With the number of people needing palliative care growing rapidly in the coming years, we are determined to be part of the answer for this significant need through our Palliative Care Strategy.

Celebrating the work of our team

During National Palliative Care Week, we take the time to recognise the amazing work of our teams across all our health and aged care services who are committed to providing the best care for people with life-limiting illness. 

This includes specialist palliative care services in our three hospitals and community based health services, and the delivery of palliative care and support through our residential care, home care and dementia services.

Our teams provide physical, spiritual and emotional care, as well as advice and referrals, to people at the end of life, and their families and carers.

Their dedication is honoured and recognised, not just this week but every day, as they work tirelessly to ensure that people at the end-of-life have access to quality care at the right time, wherever they are.
