Admissions Team
This was your first point of contact with us. Our Admission Coordinators guide you through our services and the benefits we offer. They talk with you and others to understand your loved one’s needs, goals, and preferences, so the place your loved one is offered in residential care is tailored to their individual needs. They also provide information about the financial cost of service delivery.
Residential Manager
Your main point of contact, our Residential Managers are skilled clinical professionals with expertise in aged and dementia care. They empower the care team to deliver individualised care in a calm, supportive and safe environment. They oversee every aspect of the home.
Specialised Care Team
You see this team each time you visit. They support your family member with day-to-day care, like showering, making meals, and sharing in activities that your family member enjoys. And if they notice a change in how your family member is feeling or what they can do, they will talk to you and the Residential Manager.

This dedicated team plays an essential role in creating a home for your loved one. From cleaning and laundry to maintaining shared spaces, the team makes sure your loved one’s living environment is safe, fresh, and comfortable.
Registered Nurses (Specialised Dementia Advisors)
As core members of our professional care team, registered nurses bring clinical expertise to support your loved one. Working closely with the management and support team, they focus on your loved one's physical comfort and wellbeing, providing compassionate care.
Clinical Care Manager
All our Clinical Care Managers are professionally qualified and highly trained, and assist with managing medications, pain relief, wound care and more. They can also partner with other medical professionals to determine and deliver the best clinical support for your family member.

Volunteers
HammondCare was founded on the generosity of Volunteers. Our passionate Volunteers get involved in many different ways, spending valuable time with your family member sharing in activities that are meaningful to them.
Pastoral Care Coordinator
We’re proud to offer whole-person care, which includes support from our Pastoral Carers. They are there to nurture and listen to you and your family member and answer any questions you may have.
