Section 2
Tailoring individual care
Every person we care for is unique, with their own abilities, care requirements and personal needs. That’s why we focus on what matters to each individual, promoting dignity, choice and care that feels meaningful.
We also know that finding the right social environment is important in providing care. People tend to do better when they’re surrounded by others who have similar abilities and care needs.
To really get to know your loved one, we complete a unique assessment process that looks at eight key areas. We do this before your loved one moves in and continue to review over time, to make sure the care and environment are the right fit.
- Communication
- Physical abilities
- Self-help skills
- Levels of confusion
- Changed behaviours
- Social interaction
- Psychiatric observations
- Carer dependency
The full assessment has input from our skilled team and gathers insights from the resident, family members, their representatives, health professionals, and other important people in their life.
This thorough approach helps us:
- Identify the most suitable places within our cottages.
- Build a tailored care plan.
- Recommend the right services and resources to provide appropriate care.
- Plan strategies to help your family member settle in smoothly.
Alongside our clinical assessment we also build a relationship with you and your loved one so we can understand what matters to them, their values and interests, so we can provide holistic person-centred care.
