Respite care, where you need it

Our team can support people who are frail aged, need clinical support, living with dementia, or receiving palliative care.

Respite at your place...

If you receive Home Care with us, respite care at your place can be part of the package.

But even if you don't, we can still come to you - you can pay privately, apply for a home care package, or get help sooner through the Commonwealth Home Support Programme.

Take a break at our Social Club in Green Point

River Oak Social Club provides a warm environment where older people, including people living with dementia, can connect and socialise with their community.

Carmon's Story

At HammondCare we prioritise understanding the unique needs and stories of our clients to enrich their lives. One such client, Carmon, was initially reserved when it came to sharing her interests. When introduced to Angela from the Brisbane team, she preferred to continue to enjoy her own activities independently.

Angela introduced Carmon to a sticker version of colouring books, leading to a memorable bonding session filled with smiles and laughter. This experience not only strengthened their relationship but also made Carmon more receptive to Angela's visits, allowing her husband Joe to feel at ease with stepping out.

The story exemplifies our commitment to fostering genuine connections and enhancing the wellbeing of those in our care.

How to get home care assistance

Step 1

Apply for funding

Check your eligibility by calling My Aged Care on 1800 200 422 or do an online check.

If you’re eligible for a Government subsidy, you’ll complete a face-to-face assessment organised by My Aged Care.

Step 2

Meet our team

Once you’re approved, speak to a friendly, experienced member of our Support Team to discuss your care services.

You can also talk to us about private care if you need extra support on top of your subsidised care.

Step 3

Use your services

We’ll design a care plan with you and agree on what support best suits your needs. When you’re happy with it and have funding, your care will start.

Should you need to update your services for any reason, we can help.

Take a break through Respite Care

Spend a day socialising at our social clubs and enjoy a break from your normal routine. This can also give carers a well-deserved rest.

Meeting new people and experiencing activities outside of your home can be refreshing and uplifting. Our clubs have plenty of activities to enjoy in caring, friendly day centres. These activities include:

  • social events
  • board games
  • bus outings
  • crafts
  • gardening
  • helping in the kitchen and more
  • exercise sessions

You can expect home-cooked meals served fresh while catching up with friends, or making new ones. If you’d simply rather relax and enjoy the company, that’s okay too.

Hear what others have to say

"Mum is on a Level 4 package managed by HammondCare and lives with me. I can't fault their care, with wonderful, carefully picked workers who look after Mum so well. I'll say I'm pretty fussy as I'm an RN and expect high standards... It's been such a positive experience over the last 12 months, when I'd heard so many stories about other providers."

"I'm very impressed with HammondCare. I contacted five different companies at the same time and Hammond Care won hands down with their support as I navigated the process, their knowledge and advice and their responsiveness. Within two weeks I had signed up with them and support workers started immediately."

"The girls come in to help my mother with personal care and company. They are all wonderful and beautiful people. She looks forward seeing them each week. "HammondCare allowed my father to stay at home until the very end. Eliza and the carers went over and above. They did everything for Dad and worked around his routine, going out of their way to find him at home and clean and chat with him. Eliza visited him in hospital. I am forever grateful that they were there for him."
