Carmon's Story
At HammondCare we prioritise understanding the unique needs and stories of our clients to enrich their lives. One such client, Carmon, was initially reserved when it came to sharing her interests. When introduced to Angela from the Brisbane team, she preferred to continue to enjoy her own activities independently.
Angela introduced Carmon to a sticker version of colouring books, leading to a memorable bonding session filled with smiles and laughter. This experience not only strengthened their relationship but also made Carmon more receptive to Angela's visits, allowing her husband Joe to feel at ease with stepping out.

How to get home care assistance
Apply for funding
Check your eligibility by calling My Aged Care on 1800 200 422 or do an online check.
If you’re eligible for a Government subsidy, you’ll complete a face-to-face assessment organised by My Aged Care.
Meet our team
Once you’re approved, speak to a friendly, experienced member of our Support Team to discuss your care services.
You can also talk to us about private care if you need extra support on top of your subsidised care.
Use your services
We’ll design a care plan with you and agree on what support best suits your needs. When you’re happy with it and have funding, your care will start.

Our team can help with practical, day-to-day tasks such as meal preparation, grocery shopping, and keeping your home and garden tidy and safe.
Nursing & clinical care
Our professionally qualified, highly-trained Clinical Care Managers can come to your place and assist with managing medications, wound care, post-op care, injury support and more.
Positive ageing
Our team of Physiotherapists, Occupational Therapists, Dietitians, and Exercise Physiologists can come to you, or connect you with recommended professionals in our network.
Respite care
When extra support is needed, our experienced respite team can step in to help at your place, in the community, or at one of our cottages, whether overnight or during the day.
Pastoral care
Our Pastoral Care team includes skilled and compassionate people who offer comfort, reassurance, assistance and support for people of all faiths and none, and diverse cultural backgrounds.
Volunteers
Whatever you love, there’s probably a HammondCare Volunteer who loves it, too. Our 650+ Volunteers are excited to spend time with people like you, especially around your hobbies, habits, activities and interests.

Each member of our home care team has been hired "heart-first", so whether they're providing day-to-day support around the home, companionship on social outings or clinical expertise, you won't just know you're being cared for, you'll feel it, too.

To assist with this, you will have your own personalised care plan that’s tailored specifically to you and your needs. It outlines the services you require and how often you need them. This plan will be continually reviewed, and as a result, your care plan can evolve to ensure you’re always getting the care you need.
