Understanding your home care services

As your Home Care provider, our goal is to always provide you with the most important and relevant information about the services you can receive, so you get the right help, when you need it.

Your Home Care Package is intended to give you flexibility and choice in the delivery of the care. Your care plan is personalised to your needs and details the services we provide to support you. As your care needs change, your care plan can always be amended to accommodate your needs and circumstances. You can ask your Care Manager to review your care plan at any time.

What goals are important to you?

Personal goals are an important part of your care plan. Being clear about your goals helps you choose the right care and services, and helps us to provide them for you. Good questions that can clarify your goals include:

  • What do I most want to achieve through the care that I receive?
  • What do I enjoy doing most?
  • What support do I need to feel safe in my own home?
  • Which situations do I find I need extra support?
  • To do what I want to do, what other support do I need?

What is included in your Home Care Package

In January this year, the Australian government made some important updates to the delivery of Home Care Packages in Australia, including specific changes to what is included and excluded within your package funding.

To help you better understand which care services are still included in your Home Care Packages we’ve outlined them below: 

Personal services: This includes bathing, showering, personal hygiene, grooming and help with mobility issues such as getting out of bed.

Therapies to help with mobility and clinical services: Including individual attention with speech therapy, occupational therapy and hearing and vision services.

Nutrition, hydration, meal preparation and diet: Assistance with preparing meals, including dietary requirements for health, religious, and cultural reasons. This also includes assistance with mealtimes using utensils and eating aids, and access to meal delivery services.

Nursing: Help with wound care, medication, and advice for self-care.

Continence management: Assistance in using continence aids and appliances and managing continence.

Content mobility and dexterity: Providing crutches, quadruped walkers, walking frames, walking sticks and wheelchairs. This also includes mechanical devices for lifting, bed rails, slide sheets, sheepskins, tri-pillows, and pressure relieving mattresses. Your package can also fund assistance in using the above aids.

Transport and connecting with community: Assistance with getting out into the community for shopping, attending appointments or social events. We can even provide help setting up phone or internet connections.

These recent government changes may require a review of your current Home Care Package services. If a change to your Home Care Package service is required, your Care Manager will be in contact to review your Care Plan.
