This year marks an important milestone for the Hammond Foundation as we celebrate 15 years of connecting need to care.
Established in 2011, the Foundation was created with a simple but powerful purpose: to help ensure that people facing complex health challenges, social disadvantage and end-of-life journeys have access to opportunities, experiences and support that go beyond what is otherwise available.
Fifteen years later, the impact of that vision can be seen in thousands of lives.
Thanks to the generosity of donors, supporters, community groups, trusts, foundations and corporate partners, the Hammond Foundation has raised more than $46 million since its inception. Those funds have helped improve the lives of more than 20,000 vulnerable people and their family members, creating moments of comfort, dignity, independence and connection when they were needed most.
Over the years, donor support has funded practical assistance for 285 low-income home care clients through the Care Fund, helping people remain safe and independent in their own homes. More than 550 pieces of specialised equipment have been provided, supporting comfort, mobility and quality of life for people living with complex health needs.
Supporters have also helped fulfil 277 Dreams for people approaching the end of life through the Dreams Program. Whether reconnecting with loved ones, revisiting cherished places or experiencing something meaningful for the final time, these Dreams have created lasting memories for individuals and families during some of life's most significant moments.
The Foundation's impact extends beyond individual programs. Donor support has contributed to transformative initiatives that are shaping the future of care, including HammondCare Darlinghurst, one of Sydney's few specialist aged care residences for people at risk of experiencing homelessness.
Projects such as these demonstrate the vital role philanthropy can play in helping address some of the most complex challenges facing our community.

While so much has been achieved over the past 15 years, the need continues to grow. As Australia's population ages and demand for dementia care, palliative care and support services increases, the Hammond Foundation remains committed to creating opportunities for people to access the care, experiences and support that can make a meaningful difference.
At the same time, it continues to invest in research and innovation that will help shape the future of care for generations to come.
Every milestone reached over the past 15 years has been made possible by people who chose to help. Whether through a donation, a gift in a Will, attending an event, volunteering time or advocating for HammondCare's work, supporters have played a vital role in improving the lives of thousands of people and families.
