Last Days Program

Improving the experience of families and communities in the last days of life

HammondCare and Sydney North Health Network have partnered to develop an education course, which provides skills and confidence to help a person in the last months, weeks and days of their life.

The Last Days Program aims to provide simple and effective education to carers, family, friends and community about palliative care. The program will enhance public discussion about death and dying by improving awareness, creating engagement and strengthening the community’s ability to be a more death literate society.

The education course aims to:

  • Increase skills, confidence and provide resources to support and assist loved ones and their family in the last weeks and days of life
  • Encourage open discussion about death and dying
  • Encourage communities to seek support and be active in making and documenting end-of-life choices for individuals
  • To provide a place to reflect and discuss death and dying, breaking down notions that preparing for end of life is technical, private and hidden

The course will consist of four short modules delivered to members of the community to improve awareness of palliative care. The course has been developed in consultation with community members with life experience and will be delivered by specialist palliative care clinicians and educators.

As a leader in palliative care, HammondCare is both committed and well equipped to develop this program, which will be initially piloted in Northern Sydney. Leveraging from the recognition of an ageing population within the community, including emerging public health approaches such as compassionate communities and Dying to Know Day, the program aims to improve the experience for all families at end of life.

Due to COVID-19 we had to delay the pilot dates for the Last Days program, which were scheduled for earlier this year. We are, however, pleased to announce that new dates have been set for four pilot sessions to be offered in Northern Sydney in September and November 2020. We look forward to updating you on the evaluation of these sessions and plans to roll out the program in early 2021.
