Last Days Program

Improving the experience of families and communities in the last days of life

HammondCare and Sydney North Health Network partnered to develop an education course, which provided skills and confidence to help a person in their last months, weeks and days of their life.

The Last Days Program provided simple and effective education to carers, family, friends, and the community about palliative care by delivering four workshops. The program allowed for increased public discussion about death and dying by improving awareness, creating engagement and strengthening the community’s ability to be more death literate.

The education course focused on:

  • Increased skills, confidence and provided resources to support and assist loved ones and their family in the last weeks and days of life
  • Encouraged open discussion about death and dying
  • Encouraged communities to seek support and be active in making and documenting end-of-life choices for individuals
  • Provided a place to reflect and discuss death and dying, breaking down notions that preparing for end-of-life is technical, private and hidden

An experienced HammondCare palliative care nurse delivered the program pilot in Northern Sydney between Sept 2020 and March 2021.

Leveraging the recognition of an ageing population within the community, including emerging public health approaches such as compassionate communities and Dying to Know Day, the program aimed to improve the experience for all families at end of life.
