Palliative care is a comprehensive service for those diagnosed with a life-limiting illness, when relief from symptoms or pain management may be required.

Our aim is to provide physical, spiritual and emotional comfort, skilled symptom control and good communication, as well as support for you and your family. HammondCare has a dedicated team who take the time to get to know you and your family personally, and listen to your wants and needs to determine the best care for you. Where can I access this care?

Palliative Care is provided at a variety of our locations. Inpatient Units are at Neringah Hospital, on Sydney's Upper North Shore, and Greenwich Hospital, on Sydney's North Shore, and Braeside Hospital in Sydney's South West. We also offer community services in the home, at clinics and residential aged care facilities. Based on Sydney's North Shore and Northern Beaches.

How do I refer?

A referral from a general practitioner or treating specialist is required for admission to our Palliative Care service. Usually you will then be assessed by a palliative care clinician, and admitted into the appropriate service as required.
