Your multidisciplinary palliative care team

In HammondCare's inpatient units, patient care is shared by a multidisciplinary team, who are experienced in the alleviation of a wide range of symptoms.

Who provides this service?

Your dedicated HammondCare team will get to know you and your family personally, bringing together complementary aspects of care to improve your daily comfort and wellbeing. The team may include the following specialists who will determine the best combination of care for you:

  • Doctors, Nurses and Pharmacists ensuring best practice symptom management to improve quality of life
  • Physiotherapists assisting you to maximise both your mobility and independence
  • Social Workers giving you, your carers and family information, support and counselling whenever you need it
  • Clinical Psychologists helping you and your family manage emotional hardships that may arise from diagnosis
  • Pastoral Care Coordinators providing a friendly, non-judgemental person to be with and talk with, in a safe and caring environment
  • Bereavement Services offering counselling, group support and a range of support
  • Occupational Therapists working with you to help you accomplish essential day-to-day tasks

How do I refer?

A referral from your general practitioner or treating specialist is required for admission to our Palliative Care service. For admission into an Inpatient Unit, you will be assessed by a palliative care clinician and transfers will be arranged as needed.
