Accessing home care support options

There are a range of home care support options available to meet your changing circumstances

Whether the support you already receive is self or Government funded, your care needs may change over time. So, it’s important to know how to get additional support when you need it - so you can continue to enjoy living an independent lifestyle in your own home.

You may already be using one of these funding options, but did you know your local HammondCare At Home care team can support you as your needs grow or change.

Commonwealth Home Support Programme (CHSP)
Eligibility is assessed through My Aged Care.

Government funded, entry level care for clients requiring low level care support for day-to-day living. Recommended if you only need one or two services.

You can attend one of our local social clubs with CHSP funding where you can enjoy a day out relaxing and socialising with people your own age, participate in activities and enjoy delicious home cooked meals.

Or maybe you are seeking a rest from your usual routine, so why not book yourself in for an overnight stay at one of our local respite cottages? You’ll receive guest treatment while taking time out, with all your care needs catered for.

Home Care Packages
Eligibility is assessed through My Aged Care.

Government funded, providing more support and a number of different services when your care needs become more complex and/or if you need more support on an ongoing basis. There are four levels to a Home Care Package and the funding amount increases with each level to allow you to access more services and service hours.

As your needs change you will need to apply for a new eligibility assessment with My Aged Care to determine the new funding level you are eligible for. We then work with you to tailor your Home Care Package to your care needs and what is meaningful to you.

As well as support with house cleaning and laundry, we can also assist you in getting out into the community, shopping or attending appointments. Or you may require more specialised clinical services to support your wellbeing such as physiotherapy or exercise therapy - which we can also provide at your home.

Please note - the time it takes to receive My Aged Care approval and your referral code can vary depending on the length of the national package queue. This can be between 3 – 6 months, though you may be able to receive an interim level of funding sooner, for example, the Commonwealth Home Support Programme (CHSP). In the meantime, you can contact us on 1800 826 166 to talk through your options for interim care services.

 

Private Care
Eligibility is NOT assessed through My Aged Care.

Completely self-funded option, providing support ranging from low to high level care. Costs will vary depending on the personal needs of each client.

We can support you with the same services that you would get through a Home Care Package. A reminder, we can also provide you with private care services as you wait for the approval of your Home Care Package, including help with bathing or hygiene, taking medications, nursing, or accompanying you while shopping.
