Palliative care education in home care

HammondCare offers palliative care education and training for staff caring for people with a life-limiting illness in the home, under the direction of the Palliative Care Home Support Program.

Palliative Care Home Support Program Care Worker Training

To be eligible to work with patients as part of the Palliative Care Home Support Program, care workers must have undertaken training specific to the task which is provided by HammondCare. Care workers are presented with the Fundamentals of End of Life Care in order to prepare them with the confidence and knowledge for this very important work. Four modules are presented over two days and care workers are provided with a training manual to take home for future reference.

The course outline is listed below:

  • Module 1: Essence of palliative care and communication in end of life care
  • Module 2: Ethical issues in palliative and end of life care
  • Module 3: Pain and symptom management and care during the last days
  • Module 4: Grief and loss and self care

Eligibility criteria apply. Priority will be given to training care workers in towns/regions where there is an identified shortfall of palliative care trained care workers. Training will only proceed if sufficient numbers have been confirmed two weeks prior to each session.
