About the role

The role of the Dietitian is to work within HammondCare’s expanding Restorative Care Service and within the Mission and Mission in Action of HammondCare to ensure a high standard of Dietetics service is provided to clients, along with appropriate support and advice to their families and carers. The Dietitian will participate as a member of the inter-disciplinary team (IDT) to ensure a person-centred, evidence-based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare and the Dietitian’s Association of Australia. 

HammondCare Rehabilitation and Aged Care programs include (but are not limited to) time-limited inter-disciplinary support for people who are transitioning from a hospital setting back to a home setting.  Programs are designed to maintain and / or improve physical, cognitive and psycho-social function for the individual. Programs are delivered with a restorative care focus, that is, to optimise a person’s functional capacity for activities of daily living that are important to them and make recommendations regarding any long-term care arrangements that may be required.      

The essence of this role:

  • Provision of effective and efficient Dietetic service to clients, and appropriate support and advice to their family, carers and other HammondCare staff
  • Assist clients in maintaining optimal function and well-being and where appropriate, in restoring lost function
  • Participate as a member of the IDT in assessing clients’ care needs, setting care goals and providing appropriate treatment and interventions
  • Participate as a member of the HammondCare team in the application of the organisation’s Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment
  • Participate in quality projects as part of the Allied Health team
  • Provide clinical supervision for allocated students

About you


  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
  • A tertiary qualification in Nutrition and Dietetics with eligibility for full membership to the Dietitian’s Association of Australia. APD status through Dietitian’s Association of Australia
  • Significant practical experience or extensive knowledge in providing Dietetics services in a Rehabilitation and or Aged Care setting
  • A current NSW driver’s licence, access to your own car and safe driving record
  • Willingness to travel between HammondCare’s sites and services
  • Demonstrated effective communication and interpersonal skills (written and verbal)
  • Demonstrated organisation and time management skills
  • Demonstrated ability to work independently and as part of an inter-disciplinary team
  • Demonstrated empathy and patience


  • Experience across outpatient and community settings
  • Strong administrative and computer skills
  • Experience working in a multi-cultural environment
  • An understanding of Work Health and Safety policies and procedures
