Care Team Manager - what's involved in the role?

  • Full time opportunity in the Northern Sydney HammondCare at Home Team.
  • The role of the Care Team Manager (HammondCare At Home) works within the Mission and Mission in Action of HammondCare to ensure that staff have a good understanding of individual client needs and preferences and are able to provide services tailored to the needs of individual clients, delivered within budget and service expectations.
  • The Care Team Manager is a responsible for leading, supporting, and mentoring their team, and also works in partnership and consultation with the Care Manager, Clinical Care Manager, Client Liaison Manager, and Regional Area Manager, where required.
  • Our Care Team Managers provide professional and competent assurance there are enough capable and aligned staff in your team to serve our clients.
  • You provide professional, capable and aligned leadership for your team in support of HammondCare’s Mission and Mission in Action

So, what do you need to apply?

  • Certificate IV in Aged Care (Desirable) & a minimum of 2 years relevant industry experience.
  • Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.
  • Experienced managing a team and operating effectively in a multi-site environment.
  • A current driver’s licence and safe driving record. Willingness to travel between HammondCare’s clients, sites, and services (salary package can include a packaged motor vehicle on request)
  • Strong organisational, administrative and computer skills.
  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • Empathy and patience.
