Client Liaison Manager, what's involved in the role?

The role of the Client Liaison Manager (CLM) is to be the initial first face to face contact with HammondAtHome partners in care. They are responsible for articulating to prospective partners in care, the services and benefits HammondCare offers and assess suitability for Home Care Packages, HACC, NRCP and/or HammondAtHome Private Services. This position is in our St Leonards office, working within the Northern Sydney team.

  • Create streamlined and timely processes in order to understand client needs, goals and preferences and admit them to HammondAtHome services which best meet their needs.
  • Ensure clients understand the service delivery and financial costs associated with the provision of HammondAtHome services.
  • Promote effective communication with clients, carers and family.
  • Liaise closely with other HammondCare services and external related organisations in local clinical and social networks (including GPs, ACATs, Connecting Care Teams, Medicare Locals and other referring agencies) to facilitate clear referral pathways.
  • Actively manage service waiting lists by maintaining relationships and networks with clients on waitlists, providing regular updates throughout transition to service admission.
  • Establish client agreements where clients clearly understand services and associated financial costs before service agreements are signed and care delivery commences.
  • Initiate service start-up, including setting up client’s record and task profile in client management software.
  • Collaborate with other internal and external health professionals for the optimal provision of services to meet clients’ needs, as appropriate.
  • Work cooperatively with Area Manager, Clinical Management Adviser and Case Managers to ensure a smooth transition for clients during the process of admission to service.
  • Knowledge of local area including but not limited to: geography, demographics, local health resources, clinical and care networks.

So, what do you need to apply?

  • Demonstrated skills and experience in comprehensive assessments in an aged care community setting.
  • Demonstrated knowledge of the; Aged Care Act, Home Care Packages Program Guidelines, and Home Care Common Standards.
  • Experienced in operating effectively in a multi-site environment.
  • A current driver’s licence and safe driving record. Willingness to travel between HammondCare’s sites and services.
  • Demonstrated time management and organisational skills to effectively plan and prioritise
  • Ability to maintain confidentiality of client and care worker information
  • Strong administrative and computer skills
  • Ability to work independently and as part of a team
