More about the role

The role of the Community Care Worker (Palliative Care Home Home Support) works within the Mission and Mission in Action of HammondCare to deliver a range of “in-home” services to patients who are in the last few weeks of life. The Care Worker (Palliative Care Home Home Support) works mostly alone in the patients’ homes*, but under the direct supervision of the Team Leader. This role is expected to deliver outstanding levels of patient service and to perform delegated duties to a high standard within the HammondCare Mission and Core Values so as to maximize patient satisfaction.
* home can include residential aged care facilities if client/patient place of residence

The essence of this role:

  • Deliver a range of ‘in-home’ palliative services according to the care requirements and briefing notes of each patient.
  • Provide assistance with activities of daily living, personal care, light house work, basic nursing care to maintain comfort and reassurance and support of main carer.
  • Abide by all HammondCare Workplace Health & Safety Policies and Procedures, including reporting of hazardous procedures and work areas and anything that has or might compromise the care of the patient or the health of the staff member.

About you

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • Experience in providing care support to people in the last weeks and days of life is desirable (but not essential or required).
  • This role is for someone truly dedicated to providing comfort and personal care in the home.
  • Qualification in Certificate III Aged Care (Specific Palliative Care training and workshops will be provided as part of orientation to the position).
  • Evidence of regard for elderly people, experience in providing care in the community is an advantage.
  • Experienced in operating effectively in a multi-site environment.
  • A current driver’s license and safe driving record. Willingness to travel across suburbs within Northern Sydney and South East Sydney.
  • Willingness to meet the vaccination requirements of the NSW Ministry of Health and also obtain a Working with Children Check
  • To work independently and as part of a team.
