We have a relationship-based approach to care.  This means that our community care workers take the time to fully understand the individual needs of every person we care for. In this role, you’ll enable our clients to maintain their sense of wellbeing in their own home.

 

About the role

As a Care Worker (HammondCare At Home) you’ll deliver a range of “in-home” aged care services to clients who are in receipt of a Home Care Package or other aged care services (i.e. CHSP In Home Respite, Private Service etc). The Care Worker (HammondCare At Home) works mostly alone in the clients’ homes, with regular support of their Care Manager.

In this role you’ll be expected to deliver outstanding levels of client service and to perform delegated duties to a high standard within the HammondCare Mission and Core Values so as to maximise client satisfaction.

You will

  • Deliver a range of ‘in-home’ and/or other aged care services according to the care requirements and care plans of each client.
  • Enable each client to have the opportunity to succeed at the activities of daily living which promote dignity, self-respect and maintain the client’s quality of life and abilities. This includes assisting with personal care, medication management, socialisation, shopping, housework etc.
  • The role is varied & fulfilling and one in which you really have the opportunity to ‘make someone’s day’.
  • Comprehensive & ongoing training and development provided.

 

About you

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • No experience in Aged Care is required except a passion & enthusiasm to care for the elderly in their home.
  • Basic knowledge of (or willingness to gain knowledge of) people living with dementia and their needs and a willingness to build relationships with people who have dementia.
  • Flexible in your approach to work & hours, & enjoy a degree of responsibility.
  • A current drivers licence and safe driving record. Willingness to travel between HammondCare’s sites and services.
  • Experience using a smart phone.
  • Ability to work independently and as part of a team.
  • Your most valuable qualities will be kindness, common sense & patience.
