About the role

The role of the Clinical Care Manager (HammondCare At Home) works within the Mission and Mission in Action of HammondCare to support the delivery of Home Care Package services by providing high quality clinical guidance and direct clinical care. The role has clinical oversight of individual client needs and preferences, and assists to ensure that staff are able to provide services tailored to the needs of individuals. The Clinical Care Manager is also responsible for attending clinical visits to our clients in their own homes.

The Clinical Care Manager is responsible for assessing, onboarding and reviewing clients, and  supporting Care Managers and Care Workers with the clinical management of our Home Care clients. The role has a level of professional accountability and responsibility in line with the registration requirements of a Registered Nurse (Division 1, General) from the Nursing and Midwifery Board of Australia, as the role is responsible for the management and direct care of client clinical needs.

The essence of this role:

  • Responsible for assessing and reviewing Home Care clients;
  • Responsible for the management and direct care of clients’ clinical needs;
  • Responsible for delivery of clinical education for both staff (within scope of practice) and clients/carers;
  • Responsible for management of clinical risk, liaising with other health professionals and the wider team to gain positive outcomes.

  • Clinical qualification and current registration with AHPRA including:
    • Registered Nurse - Division 1, General
    • New Graduates will also be considered, as full training and support is provided
  • A current driver’s licence and safe driving record. Willingness to travel between HammondCare’s clients, sites, and services (salary package can include a packaged motor vehicle on request).
  • Experienced in operating effectively in a multi-site environment.
  • Strong administrative and computer skills.
  • Knowledge of the Home Care Packages Program and the Aged Care Quality Standards.
  • Demonstrated ability to work independently.
  • Demonstrated ability to work autonomously and effectively work with other disciplines.
