About the role

The role of the Residential Care Admissions Coordinator works within the Mission in Action of HammondCare to be the initial face-to-face contact with HammondCare Residential services for prospective residents and their representatives.  In partnership with the residential managers, they are responsible for articulating HammondCare’s Mission, Motivation and Mission in Action to prospective partners in care, communicating clearly the services and benefits HammondCare offers. The Residential Care Admissions Coordinator will work with internal and external stakeholders to facilitate a streamlined and timely process to assist in understanding resident needs, goals, and preferences and admission to services which best meet their needs.  In partnership with the service managers, this will require the Residential Care Admissions Coordinator to network externally to HammondCare ensuring that HammondCare’s services are well known in the community, liaising with local GP’s, home care services (including HammondCare’s Dementia Specific Home Care Services), hospitals and other potential client sources to ensure a pipeline of referrals are being made and that a waitlist is maintained.

In supporting the residential managers, the role will work to ensure that the residents and/or their representatives understand the service delivery and financial costs associated with the provision of Residential services. The Residential Care Admissions Coordinator will provide the necessary information to the residential management team to ensure the best possible decision can be made with respect to admission of new residents.

The essence of this role:

  • Articulates HammondCare’s differentiated services to prospective residents and their representatives, communicating clearly the services and benefits HammondCare offers.
  • Ensure that applicants / representatives understand the costs associated with moving into residential aged care, and the different payment options that are available to them
  • Provide all necessary information to the Residential Managers to ensure the best possible decision can be made with respect to admission of new residents.
  • Create streamlined and timely processes in order to understand resident needs, goals and preferences and admit them to HammondCare Residential services which best meet their needs.

Selection Criteria;

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • Demonstrated ability to promote services across wide referrer groups
  • The ability to engage with external stakeholders and develop relationships to better promote HammondCare.
  • A solid working knowledge of the Aged Care Act (desirable)
  • Experience and knowledge of the Aged care fees & charges & be able to apply this to the preparation of resident agreements
  • Significant demonstrated financial acumen (desirable – although a willingness to learn would be considered)
  • Strong administrative and computer skills.
  • Ability to work independently and as part of a team.
  • Empathy and patience.
  • Excellent communication skills, both written & oral
  • Capacity to relate to a diverse range of people
  • Drivers licence & ability to travel to network with potential referrers
