About the role

The role of the ACFI Coordinator is responsible for working with HammondCare’s Residential Care Homes to collect and bring together all documentation in preparation of ACFI submissions in a timely manner.  The ACFI Coordinator will also be available to review resident ACFI claims and classifications to ensure optimum funding for the facility.  In addition, they are required to provide support and mentor all team members and work closely with the Residential Manager to ensure all ACFI submissions are in line with required standards and HammondCare Mission and Values. 

The ACFI Coordinator is required to provide a high level of service to enable the facility to function smoothly and efficiently. It is an important role that requires commitment, initiative and confidence dealing with all stakeholders. The workplace is flexible and friendly, with an emphasis on reciprocal support between management and staff.

The essence of this role:

  • Working with the Waratah team to ensure efficient and effective processes are in place plus to maintain ACFI submissions from admission to discharge.
  • Mentoring staff including Admission Coordinators, Managers, Registered Nurses and Care Service Employees to complete accurate documentation to support ACFI whilst maintaining our model of care.
  • Ensure all ACFI Packs are “validation” ready and are defendable. 
  • Submit all ACFI claims once reviewed with the Residential Manager.


About you (Selection Criteria)

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • A solid working knowledge of the Aged Care Funding essential
  • Strong administrative and computer skills.
  • Ability to work independently and as part of a team.
  • Empathy and patience.
  • Excellent communication skills, both written & oral
  • Capacity to relate to a diverse range of people
  • Drivers licence
