The original Hierarchic Dementia Scale-Revised (HDS-R) Kit comes with 20 scoresheets. Additional HDS-R Scoresheets Pack can now be purchased with each pack consisting of another 20 scoresheets to enable continued use of the kit.
The additional HDS-R scoresheets match the originals and exactly and are in the convenient rolled-fold A4 format. Further replacement components will be available shortly - let us know if you have any requirements.
