What is a Home Care Package?
A Home Care Package is a coordinated package of services tailored to meet a person’s care needs. Home Care Packages provide people with an alternative to moving from their home to a residential care home. We will meet with you and develop a care plan tailored to your individual needs. This will allow us to provide a range of care services that can help you to remain your own home for as long as possible. Levels of care differ depending on assessed need.
How do I get a Home Care Package?
If you think a Home Care Package might be helpful for you or someone you know, the person needing care first needs to be assessed by the local Aged Care Assessment Service (ACAS), contactable through your doctor or local hospital. You can also access contact details for your local ACAS by contacting My Aged Care on 1800 200 422 or by visiting www.myagedcare.gov.au.
Referrals to the ACAS may come from the people requiring care themselves, their family or a doctor.
Any person who is having difficulty managing at home may be eligible for a Home Care Package. For HammondCare Dementia- Specific Home Care Services, the care recipient requires a diagnosis of dementia to be eligible. If assessed as eligible, and packages are available, a needs-based assessment is conducted by one of HammondCare’s Home Care Service Managers who will work with you to tailor the package to meet your individual needs.
How can we help?
Our Home Care Service Manager will:
- Visit you at home to discuss your individual needs
- Work with you to develop a suitable care plan
- Talk with your family and friends, if desired
- Liaise with other services, as required
- Find and implement creative solutions
- Arrange ongoing support, to help you to remain at home for as long as possible.
The support we will provide
Having discussed your needs with you, we will work with you to develop a care package suitable for you, within the scope of the program.
A Home Care Service care worker may visit you at home and assist with day-to-day activities such as personal care, domestic assistance and transport to medical appointments. If you require more extensive care, the Home Care Service Manager may refer you to a higher level Home Care Package, your GP, local hospital, or allied health services.
